Room Request Form

This form must be submitted and approved prior to using space in Faith Lutheran Church.

This form is required so that we can avoid conflicts and make sure that we've got your space available and ready when you need it. We will process your request and let you know when it's been approved.

By church policy, we give priority to requests in this order:

All congregational services of worship, regularly scheduled teaching activities and funerals
Wedding schedule 3 or more months in advance
Church committees, functions and activities
Other programs sponsored by the church
Other community related programs

By submitting this form, you agree to respect and observe the traditions and position of our people and pledge not to smoke, use alcoholic beverages or drugs, or use profane language in the church building.
